摘要
目的探讨老年阵发性房颤(PAF)患者的左心房大小及P波离散度的变化,并进行相关性分析。方法选取52例有PAF发作史的老年心脏病患者与55例无心房颤动发作史的老年患者,分别测量其窦性心律时的12导联心电图(ECG),并分别测定最大P波时限(Pmax)及最小P波时限(Pmin),并计算P波离散度(Pd);心脏超声测量2组患者的左心房内径(LAD)、左室射血分数(LVEF)。结果PAF组的Pmax、Pmin及Pd值分别为:(118·56±12·63)ms、(70·82±8·69)ms及(47·74±6·78)ms;而对照组分别为:(112·53±10·03)ms、(71·47±6·78)ms及(40·82±5·26)ms,两者相比,PAF组中Pmax、Pd较对照组明显延长(P<0·05);此外,2组在进行心脏超声测量中,PAF组的LAD值(4·19±0·38)cm较对照组(3·58±0·26)cm相比,有显著差别(P<0·05)。结论Pd、LAD与PAF的发生有密切的联系。
Objective To study the relationship of P wave dispersion and left atrial dimension in elderly patients with paroxysmal atrial fibrillation (PAF). Methods The maximum P wave duration (Pmax) , the minimum P wave duration (Pmin) and P wave dispersion (Pd = Pmax- Pmin) on 12 leads electrocardiogram and left atrial dimension (LAD) and left ventricular ejection function (LVEF) on the echocardiogram were measured in 52 elderly patients with PAF( PAF group) and 55 elderly patients without history of atrial fibrillation ( control group). Results Pmax and Pd were significandy higher in PAF group than those in control group ( Pmax: 118. 56 ± 12.63 versus 112.53 ± 10. 03 ms, Pd: 47. 74 ± 6. 78 versus 40. 82 ±5.26 ms, P 〈0. 05) ; LAD was significantly higher in PAF group than that in control group (4. 19 ± 0. 38 versus 3.58 ± 0. 26 cm, P 〈 0. 05 ). Conclusions The change of Pd and LAD was associated with the recurrence of PAF in elderly patients.
