摘要
目的:探讨猪单侧肺移植术中理想的麻醉管理方法。方法:选择心肺功能良好的14只猪作为实验动物,体重接近者配对作为供、受体进行单肺移植,采用静脉复合麻醉,气管插管接呼吸机行机械通气,保持液体平衡,支持循环。移植后降低吸入氧浓度,并用低潮气量通气。于不同时间点检测受体猪血氧饱和度(SpO2)、肺通气阻力(LR)和血压(BP)变化。结果:单肺移植后各时点SpO2、LR及BP变化与受体自体肺移植切除前比较无明显变化(P<0.05),测定结果表明植入肺的功能良好。结论:麻醉前充分准备、术中全面监测、控制液体量、血管扩张药的应用等措施是猪单肺移植麻醉成功的关键。
Objective: To study the anesthetic management for single lung transplantation in pigs. Methods: Fourteen pigs with satisfactory heart and lung functions were chosen. They were divided into seven pairs, each with a similar body weight. In each pair, one was used for donor and another for recipient. All the subjects were given balanced anesthesia and mechanical ventilation. Keep fluid balance and sustain circulation. After the operation set down the fraction of inspired oxygen and tidal volume. Saturation of blood oxygen(SpO2 ), resistance of lung ventilation(LR)and blood pressure(BP) were measured at different time point with the recipient's native pulmonary artery ligated. Results: There were no significant difference (P〈0.05) of SpO2, LR and BP after opcration for recipient compared with preoperative values. The results wed that graft function and preservation were satisfactory. Conclusion: Perfect preparation of anesthe- the universal monitoring during surgery, the fluid control, and the application of vasodilators are the stepts of a successful anesthesia for a single lung transplantation.
