肠内免疫营养对胃肠道恶性肿瘤围手术期免疫功能的影响 被引量:5

Study of perioperative enteral immunonutrition in gastrointestinal cancer patients

摘要 目的评价肠内免疫营养对胃肠道恶性肿瘤术后免疫功能、炎症反应及并发症的影响,探讨临床推广应用的理论依据。方法将90例胃肠道恶性肿瘤随机分为免疫营养组、普通营养组和对照组,术前5d和术后1、8d分别检测多项免疫指标。结果术后免疫营养组免疫指标明显高于普通营养组和对照组,炎性指标明显低于普通营养组和对照组;术后免疫营养组的并发症明显低于普通营养组和对照组。结论围手术期肠内免疫营养可在术后早期提高病人的免疫功能,减轻术后应激反应,降低术后并发症的发生率,并缩短平均住院时间。 [Objective ] To evaluate if the perioperatively administrated enteral immunonutrition in gastrointestinal cancer patients will improve the early postoperative immune and inflammatory responses and decrease postoperative morbidity, [Methods] A prospective and randomized clinical trail was performed. Ninety patients with gastrointestinal cancer were randomized divided into immunonutrition group, standard-nutrition group and control group. All immunal variables were measured on preoperative day 5 and postoperative day 1 and 8. [Results] Postoperative immune variables in the immuno-group were significantly higher than in standard-group and control-group. Postop- erative inflammatory variables in the immunonutrition group were significantly lower than in other groups. In the immunonutrion group, there were sighificantly fewer patients experienced postoperative complications, [ Conclusion] The perioperative enteral immunonutrition can significantly modulate the postoperative immunsuppressive and inflammatory responses, decrease the occurrence of postoperative complicatins as well as the length of hospital stay.
