MadCAM-1与整合素β7免疫细胞浸润在食管癌发生中的作用

摘要 目的探讨M adCAM-1(m ucosal addressin cell adhesion m olecu le-1)的变化及整合素β7免疫细胞浸润在食管癌发生中的作用。方法用免疫组织化学SP法,检测50例行食管癌根治术所收集的癌组织、癌旁组织、正常食道黏膜组织中M adCAM-1与免疫细胞上整合素β7的表达。结果M adCAM-1表达率在正常食道黏膜、癌旁组织及癌组织中分别为78%(39/50),64%(32/50)和40%(20/50)。癌组织中的阳性表达率明显低于正常组织(P<0.01)和癌旁组织(P<0.01);而正常组织与癌旁组织之间差异无统计学意义(P>0.05);M adCAM-1阳性表达患者中在低年龄组(<45 y)中的表达低于高年龄组(>45 y)(P<0.05);M adCAM-1阳性表达率与TNM分型、肿瘤部位、肿瘤大小及癌细胞的浸润深度和分化程度无关(P>0.05)。M adCAM-1阳性表达组中β7免疫细胞数量癌组织中明显低于癌旁及正常组织(P<0.01)。结论M adCAM-1参与肿瘤局部免疫过程,与食管癌发生、发展有关。食管癌组织中M adCAM-1的表达减少,β7免疫细胞浸润减少。提高M adCAM-1表达是肿瘤免疫治疗的一种方法。 Objective To study the function of mucosal addressin cell adhesion molecule-1 and integrant β7 and biological behavior of invasion and metastasis in esophageal carcinoma. Methods The immunoreactivity of MadCAM-1 and β7 was revealed by immunohistochemical method in esophageal carcinoma normal esophagus mucosae ,cancer-adjacent tissues in 50 collected specimens when carried out wdical resection carcinoma of esophagus. Results The percentage of MadCAM-1 positive strong immunoreactivity was 78% (39/50),64%(32/50)and 40% (20/50) in normal esophagus mucosae,canceradjacent tissues and esophageal carcinoma respectively. The strong immunoreactivity of cancer tissues was obviously lower than normal esophagus mucosae or cancer-adjacent tissues (P〈0. 01). There were no difference between normal esophagus mucosae and cancer-adjacent tissues (P〉0. 05). The percentage of MadCAM-1 positive strong immunoreactivity was no correlation with TNM type,tumor location and tumor size (P〉0.05). 67 immune cell in group of MadCAM-1 positive strong immunoreactivity was obviously lower than normal and cancer-adjacent tissues (P〈0.01). Conclusion MadCAM-1 is an important role of invasion and metastasis for esophageal carcinoma. Increased the expression of MadCAM-1 is the way to treat esophageal carcinoma. Detection of MadCAM-1 cancer tissues can be helpful to understand the carcinogenic biological behavior of esophageal carcinoma as well as make judgment and treatment and prognosis of patients.
