Gamma钉内固定与骨牵引治疗股骨转子间骨折疗效比较

GAMMA NAIL INTERNAL FIXATION AND BONE TRACTION IN THE TREATMENT OF FEMORAL INTERTROCHANTERIC FRACTURE: A Therapeutic-effect Comparison

通过对比Gamma钉内固定与骨牵引治疗股骨转子间骨折的四项临床指标，评估这两种治疗方法的优劣，为临床在治疗上提供选择的依所。以年代划界取样，将1989～1996年间采用骨牵引方法治疗股骨转子间骨折97例设为A组；将1999—2003年采用Gamma钉内固定治疗股骨转子间骨折32例设为B组，对两组的骨折愈合时间、关节屈髋活动度达到900的时间、下床负重时间、并发症等四个方面进行比较。结果显示，骨折愈合时间A组8±0、5周，B组8±0．3周；关节活动度达到900的时间A组56±8．4天，B组20±5，5天；下床负重时间A组65±6．3天，B组46±7，5天；并发症A组43例，B组7例。表明在功能康复及预防并发症方面，Gamma钉内固定治疗股骨转子间骨折有较大优势。

The objective of the paper is to evaluate the advantages and disadvantages of Gamma nail internal fixation(GNF) and bone traction(BT) in the treatment of femoral intertrochanteric fracture(FIF) so as to offer a basis for the clinical selection of the two treatment methods.Ninety-eight cases of FIF treated by BT in 1989-1996 were taken as Group A and 32 cases treated by GNF in 1999-2003 were taken as Group B;the time for fracture healing,for reaching 90°hip flexion and for off-bed loading and the complications of the two groups were compared.The results showed the time for fracture healing: 8±0.5 weeks of Group A versus 8±0.3 weeks of Group B;the time for reaching 90°hip flexion: 56±8.4 days of Group A versus 20±5.5 days of Group B;the time for off-bed loading: 65±6.3 days of Group A versus 46±7.5 days of Group B;the complications: 43 cases in Group A versus 7 cases in Group B,suggesting that GNF in treating FIF has a great advantage over BT in the function recovery and the complication prevention.
