内镜诊断早期胃癌187例

Clinical presentations of early gastric cancer diagnosed by endoscopy

目的总结分析早期胃癌的临床、内镜下及病理特点,提高对早期胃癌的检出率。方法选择该院开展内镜以来,20年间胃镜发现并行外科手术后经病理证实为早期胃癌的病例187例,将其内镜下表现、病理组织学特点及随访结果加以分析。结果早期胃癌切除占胃癌切除的4.99%;电子内镜检查活检证实为早期胃癌的阳性率为93.1%(174/187);部位以胃窦居多占49.2%;I型I、I型I、II型早期胃癌检出率分别为25.7%、54.0%及20.3%。结论早期胃癌临床无特征性症状,诊断主要靠内镜医师认真细致观察和识别早期胃癌的形态改变。

[Objective] To analyze clinical, endoscopic and pathologic features of early gastric cancer. [Methods] To study endoscopic, histopathologic and follow-up findings in 187 patients in our hospital who after endoscopy and surgery have been pathologically identified as early gastric cancer. [Results] Excision of early gastric cancer accounts for 4.99% of gastric cancer. The cases of early gastric cancer are174 out of 187 patients （93.1%） by endoscopic biopsy gastric antrum： （49.2%）; type Ⅰ,Ⅱ,Ⅲ of early gastric cancer（25.7%; 54.0%; 20.3%）. [Conclusion] Early gastric cancer clinically has no characteristic manifestations and diagnosis mainly depends on careful observation in endoscopy and identification of its changes in pattern.
