四味瓜乳消渴汤治疗2型糖尿病血瘀证患者的临床观察

目的观察四味瓜乳消渴汤对2型糖尿病血瘀证患者的治疗作用。方法选择2型糖尿病血瘀证患者108例,分为两组。西药对照组48例按西医治疗方案控制血糖;观察组60例服用四味瓜乳消渴汤治疗,疗程为2个月。治疗前及疗程结束观察临床症状积分、空腹血糖(FBG)、餐后2h血糖(PBG)、糖化血红蛋白(HbA1c)、血总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、血液流变学指标(全血粘度、血浆粘度、红细胞压积、纤维蛋白原)。同时进行安全性检查。结果观察组治疗后中医症状积分下降明显,与对照组比较,差异具有显著性。观察组总有效率明显优于对照组,组间差异具有显著性。观察组治疗后FBG、PBG、HbA1c均明显下降,与对照组比较,差异具有显著性。观察组治疗后TC、TG、LDL-C明显下降,HDL-C明显升高,差异具有显著性;两组间比较,TC、TG、LDL-C差异具有显著性,HDL-C差异无显著性。观察组治疗后血液流变学指标都有明显改善,差异非常显著,两组间比较,全血粘度低切、血浆粘度、纤维蛋白原的差异显著;安全性检查未见不良反应发生。结论四味瓜乳消渴汤能改善中医症状积分,提高中医症状疗效,有良好的调节脂类代谢与改善血液流变学指标作用。提示四味瓜乳消渴汤对于2型糖尿病的治疗具有多种作用。

Objective To study the clinical efficacy of Siweignaruxiaoketang decoction for the treatment of type 2 diabetes with blood-stasis syndrome. Methods 108 cases were divided randomly into two groups. 60 cases in the treatment group were treated with oral Siweignaruxiaoketang decoction, and 48 cases in the control group were treated with chemical（western） medicine, for a course of 2 months. The changes of clinical symptoms, and fasting blood glucose（FBG）, 2 h postprandial glucose（2 hPG）, glycosylated hemoglobin（HbA1c）, total cholesterol（Tc）, triglycerides（TG）, high-density lipoprotein（HDL-c）, low-density lipoprotein（LDL-c）levels, and haemo-rheology parameters were analysed. The safety of the medicine was evaluated also. Results After treatment,the clinical symptomatic scores were decreased significantly in the treatment group, compared with the control group. The levels of TC, TG, LDL-C obviously descended, and HDL-C ascended in the treatment group. There was very significant differences in TC, TG, LDL-C. Compared with pretreatment, haemo-rheological parameters in the treatment group obviously improved. The levels of whole blood viscosity at low-shear rate（10 sec）, plasma viscosity, fibrinogen were significantly different, There was no significant side-effect in the treatment group. Condusion The results obtained suggest that Siweiguaruxiaoketang decoction can improve the clinical symptomatic integrals and remarkably enhance total clinical efficacy, and can control the obstinate hyperglycemia,and improve fat metabolism as well as hemo-rheological parameters. It has indicated that Siweiguaruxiaoketang decoction has multipurpose therapeutic actions for diabetes mellitus.
