妊娠高血压疾病患者血浆同型半胱氨酸及代谢相关因子的测定及其关系探讨

Study of Relationship between the level of plasma homocystein and Serum folic acid,vitamin B_ (12),vitamin B_6 in pregnancy induced hypertension syndrome

目的探讨血浆同型半胱氨酸(Hcy)浓度与妊娠高血压疾病发生发展的关系。方法检测48例妊娠高血压疾病患者、60例正常妊娠晚期孕妇和16例非妊娠妇女血浆Hcy和血清叶酸以及维生素B6、维生素B12含量。结果①正常妊娠妇女血浆Hcy以及血清叶酸、维生素B12、维生素B6水平较非妊娠妇女均明显下降(均为P<0.05)。②妊娠高血压疾病组与正常妊娠组相比较,血浆Hcy水平明显升高(P<0.01),高Hcy血症者妊娠高血压疾病的发病风险较正常Hcy组升高3.6倍,(P<0.05)。中重型妊娠高血压疾病组血浆Hcy水平明显高于轻型妊娠高血压疾病组(P<0.05),而轻型妊娠高血压疾病组与正常妊娠组相比二者的差异无显著性(P>0.05);血清叶酸水平明显降低(P<0.05),而维生素B6以及维生素B12水平无明显差异(P>0.05)。妊娠高血压疾病组与正常妊娠组血浆Hcy水平与血清叶酸水平成直线负相关(P<0.05),而与维生素B12、维生素B6无明显相关性(P>0.05)。结论孕妇血浆Hcy浓度升高,妊娠高血压疾病的发生风险明显增加。Hcy浓度升高与妊娠高血压疾病的发生发展有一定的关系。

Objective To investigate the relationship between the level of plasma homoeystein（Hey ） and the pregnancy induced hypertension syndrome （ PIH ）. Methods The levels of plasma Hey .serun folie acid and vitamin B12.vitaminB6 were measured in PIH group （n = 48 ） , normal pregnancy group（ n = 60 ）and normal nonpregnaney women group（n = 16）. Results （1） The levels of plasma of Hey and serum folie acid and vitamin B12, vitamin B6 in normal pregnancy group were decreased than nonpregnaney group（P 〈0.05 for each）. （2） The plasma of Hey in PIH group increased significantly than normal pregnancy group （ P 〈0. 01 ）. Pregnancy women with hyperhomoeysteinemia had 3.6-fold risk for PIH eornpared with those with normal Hey （ P 〈0. 05 ） . The Hey level in moderate and serious PIH group increased gready than in mild PIH group（ P 〈0.05 ）. There was no difference between mild PIH group and normal pregnancy group （ P 〉0. 05 ）. The folie acid level in PIH group decreased significantly （P 〈0. 05 ） , while vitamin B12 and vitamin B6 level showed no significant differences compared with normal pregnancy group （ P 〉 0. 05 ）. Correlation analysis showed that plasma Hey was negatively correlated with folle acid in normal pregnancy group and PIH group （ P 〈 0. 05 ） , and there was no clear correlation between plasma Hey and serum vitamin B12 and vitamin B6 between two groups （P 〉 0. 05 ）. Conclusion Pregnancy women with elevated Hey level have the increased risk of PIH. Hey maybe play a role in the pathogenesis of PIH.
