左布比卡因复合芬太尼腰麻用于剖宫产

摘要 目的 探讨剖宫产患者左布比卡因复合芬太尼腰麻的有效剂量及安全性.方法 120例单胎足月孕妇,ASA Ⅰ~Ⅱ级,随机分为三组:Ⅰ组为0.5%左布比卡因7 mg+芬太尼25μg,Ⅱ组0.5%左布比卡因8 mg+芬太尼25μg,Ⅲ组0.5%左布比卡因9 mg+芬太尼25 μg,每组40例.三组均于L3~4椎间隙穿刺行腰麻-硬膜外联合麻醉,蛛网膜下腔注药.结果 Ⅰ组麻醉效果分级低于Ⅱ、Ⅲ组(P<0.05).三组的最高麻醉平面,达到最高平面时间及达Bromage 3级时间差异无统计学意义(P>0.05),Ⅰ组麻醉平面消退到T12的时间及Bromage分级恢复到0级时间均短于Ⅱ、Ⅲ组(P<0.05),低血压发生率Ⅰ组<Ⅱ组<Ⅲ组(P<0.05),呼吸困难、恶心呕吐、寒战,产钳率差异均无统计学意义,新生儿娩出后1 min、5 min Apgar三组间差异无统计学意义,术后随访未发现有任何神经并发症.结论 左布比卡因有效剂量为8~9 mg,复合芬太尼腰麻可以安全地应用于剖宫产.
