摘要
[目的]观察缺血性脑血管病(ischemic cerebrovascular disease,ICVD)患者不同程度颈动脉病变血管反应性(vasomotor reactivity,VMR)的变化。[方法]应用高频彩超对90例ICVD患者和30例正常对照组人群进行颈动脉的超声检查并测定血管反应性,将各项超声指标进行成组设计的T检验。[结果]90例ICVD患者中轻、中度颈动脉狭窄组患者的血流速度指标(除中度ICA狭窄患者的MFV增高)PSV、EDV、MFV及其相关指标RI、PI均无明显改变;重度颈动脉狭窄组患者的PSV、EDV、MFV、RI、PI明显增高(P<0.01)。ICVD患者VMR均低于正常对照组(P<0.01)。随着颈动脉狭窄分级程度的加重VMR有降低的趋势。[结论]VMR是早期ICVD判定的敏感指标。通过对VMR的检测来判断脑血管舒缩功能的储备能力并对ICVD的发病风险进行预测。
[Objective] To observe the ultrasonic characteristics of carotid stenosis in patients with ischemic cerebrovascular disease (ICVD) by color Doppler flow imagine (CDFI). Study the change of vasomotor reactivity (VMR) in patients with ICVD of different degree carotid stenosis. [Method] This stuty conducted Ultrasonic examinations on 90 in and out ICVD patients and 30 health to check the index of blood flow velocity. Vasomotor reactivity was another focus. After conducting the T test on groups, we used SPSS to calculate the P value and leave those less than 0.05 for further study. [Results] ① The VMR in mild, moderate and severe carotid stenosis was all lower than that of control group. With the development of carotid stenosis, the VMR was tended to be impaired. ② The index of blood flow velocity in carotid artery severe stenosis group was significantly faster than that of control group. (P 〈 0.01) There was no significant difference between mild - moderate carotid stenosis group and control group in index of blood flow velocity. [Conclusions] VMR is a sensitive index to diagnose carotid stenosis early. To examining VMR and predict the risk of ICVD can judge the ability of vessel dilation reserve.
