摘要
目的研究77例胃肠癌患者外周血中组织特异性mRNA(CK 20、CK 19及CEA)的表达,并探讨其意义。方法RT-PCR技术检测77例胃肠癌患者术前外周血中CK 20、CK 19、CEA mRNA的表达。结果77例胃肠癌患者外周血中,CK 20、CK 19及CEA的阳性表达率分别为44.2%、35.1%及54.5%;三基因同时检测时,至少一个基因检测结果为阳性的表达率为74.0%,与肿瘤的组织分化程度、浸润深度、淋巴结转移、远处转移及复发具有相关性。结论分子生物学技术联合检测多种标志物检测肿瘤细胞的外周血微转移,可提高检测的敏感性及准确性,有助于指导临床治疗及预后判断。
Objective To investigate the histospecific mRNA (CK20,CK19,CEA) expression in peripheral blood from patients with gastrointestinal carcinoma and to evaluate its clinical significance. Methods A total of 77 preoperative peripheral blood specimens obtained from patients with gastrointestinal carcinoma were analyzed by CK20,CK19 and CEA- specific reverse transcriptase-nested-polymerase chain reaction. Results Among the 77 blood samples, the expression of CK20,CK19 and CEA mRNA was 44.2% ,35.1% and 54.5% ,respectively. 74.0% were positive for at least one markers for these three genes ,and were significantly correlated with the differentiation,depth of invasion,lymph node metastasis,distant metastasis of the cancer. Conclusion The molecular detection of circulating tumor by the use of multiple markers could help increase PCR positivity and sensitivity, and be helpful to the selection of clinical managements and the judgement of prognosis.
