大肠癌组织中HSP27、bcl-2、p53和P-gp的表达及其意义 被引量:5

摘要 目的探讨热休克蛋白27(Heatshockprotein,HSP27)和bcl-2、p53、P-gp蛋白在大肠癌组织中的表达及其意义。方法运用免疫组织化学SP法检测HSP27和bcl-2、p53、P-gp在72例大肠癌组织中的表达,并运用多因素COX比例风险模型分析患者的预后。结果HSP27、bcl-2、p53和P-gp在大肠癌组织中的阳性表达率分别为69.44%(50/72)、54.17%(39/72)、59.72%(43/72)和40.28%(29/72)。HSP27在高分化腺癌组的表达率为84.85%,显著高于低分化组的28.57%(χ2=29.614,P=0.000),bcl-2、p53和P-gp在高分化腺癌组的表达率也高于低分化腺癌组,具有统计学意义(P<0.05)。HSP27与bcl-2及P-gp在大肠癌中的表达具有相关性(r=0.380,P=0.001;r=0.284,P=0.016)。COX回归分析提示HSP27表达与生存期呈负相关。结论HSP27、bcl-2、p53和P-gp在大肠癌组织中呈高表达,提示与大肠癌耐药有关;联合检测对临床制定合理的化疗方案具有指导意义;HSP27可作为判断大肠癌预后的一个独立预测指标。 Objective To evaluate the expression and significance of HSP27 and bcl-2.p53,P gp protein in colorectal carcinoma. Methods The expression of HSP27 and bcl 2, p53, P-gp in 72 cases of colorectal carcinoma were detected by SP immunohistochemical technique. Results The positive rate of HSP27 and bcl 2,p53,P-gp protein was 69. 44% (50/72) ,54. 17% (39/72) and 59. 72% (43/72) ,40.28%(29/72), respectively. The positive rate of HSP27 in the well differentiated group was 84. 85% (28/33), while it was 28.57%(4/14)in the poor differentiated group, (Χ^2 = 29. 614,P =0. 000). The positive rate of bcl-2 , p53 and P gp was higher in the well differentiated group than in the poor differentiated group (P〈0. 05). The expression of HSP27 was related to the expression of bcl-2 and P-gp(r = 0.380, P = 0.001, r = 0. 284, P = 0. 016). The correlation with prognosis was analyzed by COX proportional hazards model. It showed that HSP27 was independently associated with patient survival length. Conclusion The high expression of HSP27 and bcl-2 ,p53 ,P-gp was correlated with the multidrug resistance (MDR) of colorectal carcinoma. The unitive examination of HSP27 and bcl-2 ,p53,P-gp may help make out a chemotherapy plan. Expression of HSP27 protein was a independent and accurate predictor of poor clinical outcome for individual colorectal carcinoma.
