新式简易法腹膜外剖宫产术206例临床分析

Clinical analysis on 206 cases with simple new method extraperitoneal ceaaren section

目的：探讨新式简易法腹膜外剖宫产术的临床效果。方法：对我院行新式简易法腹膜外剖宫产术206例（观察组）与同期随机选取新式子宫下段剖宫产术200例（对照组）做对照分析，对两组手术时间、术中出血量、术中及术后并发症、术后病率、胃肠功能恢复情况及新生儿评分进行比较。结果：手术时间观察组平均44min，对照组平均40．5min（P〉0．05），术中出血量观察组平均205ml，对照组210ml（P〉0．05）；术后排气时间观察组〈24h者125例，24-48h者81例，〉48h者为0例，对照组分别为78例、96例、26例（P〈0．01）；观察组术后病率及术后吸收热、腹胀等均明显低于对照组（P〈0．01）；两组均无切口感染、无娩头困难和产钳助产。结论：新式简易法腹膜外剖宫产术具有术后并发症少、术后病率低、恢复快、操作方法简单且不进腹腔的优点，尤其适宜在胎膜早破、羊水污染、产程延长等存在潜在感染的病例中应用。

Objective： To study the clinical effects of new method extraperitoneal cesarean section. Methods： Select randomly 206 cases with simpile new extraperitoneal cesarean section as study group, and 200 cases with low cesarean section as control group, to compare the operative duration, intraoperative average of bleeding, postoperative vectal carminative time, postoperative mobidity and recovery score between the two groups. Results： The average operating time and intraoperative bleeding of study groups were 44 min and 205ml, those in control groups were 40. 5 min and 210 ml （P 〉0. 05）, the postoperative rectal carminative time of study group： 125 cases in 24 hours, 81 cases in 24 -48 hours, none in the 48 hours, those in control groups were 78 cases, 96 cases and 26 cases （P 〈 0. 01 ） , the postoperative mobidity, postoperative obstetric forceps and abdominal distention in study groups were less than those in control group （ P 〈 0.01 ）, none wound infection, expulsive difficulty and forceps delivery cages in two groups. Conclusion： This new method operation has many advantages such less complication, lower postoperative morbidity, quicker recovery, and simple operation, it can especially be used in premature rupture of membranes, infection of amniotic cavity, prolonged labor cases.
