彩色多普勒超声在异位妊娠保守治疗中应用的评价

Usefulness of color Doppler ultrasonography in the diagnosis of early ectopic pregnancy and subsequent surveillance during the conservative treatment

目的探讨彩色多普勒超声在异位妊娠保守治疗监测中的价值。方法应用彩色多普勒超声对65例临床怀疑异位妊娠患者进行早期诊断,并对其保守治疗(甲氨蝶呤)过程进行动态监测。结果彩色多普勒超声检查对65例可疑异位妊娠患者的早期确诊率达96.92%。异位妊娠保守治疗成功35例(56%),失败28例。包块内探及血流信号者,其血β-HCG下降速度较快,保守治疗成功率在包块内有或者无血流信号两组间无明显差异。结论彩色多普勒检查对异位妊娠早期诊断及保守治疗过程的动态监测具有重要意义。

Objective To evaluate the usefulness of color Doppler uhrasonography（CDUS） in the diagnosis of early ectopic pregnancy（EEP） and subsequent surveilliance during the conservative treatment. Methods Sixty - five patients with clinically suspected EEP underwent CDUS examinations and then their concervative treatments （using MTX）were dynamically monitored by CDUS. Results Among 65 cases with early ectopic pregnancy, the diagnostic accuracy of CDUS was 96.96 % and the successful rate of conservative treatmen was 56 %. The B - HCG level in patients whose masses had blood flow signals was dropped quickly. However, there was not significant difference between the successful rate of conservative treatment and the presence or absence of CDFI signals in the masses. Conclusion CDUS examinations have important role to diagnose the EEP and monitor dynamically their conservative treatments.
