冬眠心肌动物模型的制备及其MRI表现的实验研究 被引量:1

摘要 目的探讨采用不全结扎法建立兔冬眠心肌模型的可行性以及心脏磁共振成像(CMRI)对冬眠心肌的诊断价值。材料与方法健康新西兰大白兔15只,开胸对其左冠状动脉前降支近段进行不全结扎以造成冠状动脉狭窄,在术后相应时间采用不同序列行CMRI检查,将CMRI结果与心肌病理进行对比,评价动物模型建立的成功率以及冬眠心肌的CMRI表现。结果观察15只兔的CMRI及病理结果,其中有10只兔符合冬眠心肌动物模型改变,动物模型制备成功率为66.7%。对10只符合冬眠心肌的兔CMRI分析表明,动态灌注扫描表现为病变节段心肌灌注降低,延迟增强扫描均未见明显延迟强化灶;电影磁共振成像(cine-MRI)表现为节段性室壁运动减弱,室壁增厚度降低;常规CMRIT1、T2扫描均未见明显异常改变。结论不全结扎法建立兔冬眠心肌模型具有较好的仿真性,且简单、经济、易行,是冬眠心肌动物模型建立的一种较好的方法,同时CMRI是一种评价冬眠心肌的行之有效的检查方法。 Objective To discuss the feasibility of establishing rabbit hibernating myocardium model, and to evaluate the value of CMRI for demonstrating the hibernating myocardium. Materials and Methods The proximal LADs were obstructed by part ligation to make hibernating myocardium model in 15 New Zealand rabbits. Multiple sequences of CMRI were performed at relevant time after operation. The appearances of CMRI were compared with the results of pathological results, and the successful rate was evaluated. Results The pathological results of 10 rabbits were consistent with hibernating myoeardium model; the successful rate of establishment of hibernating myoeardium model was 66.7% . CMRI appearances of hibernating myocardium showed hypoperfusion, and enhancement on delayed phase was not found during dynamic perfusion scan; Cine CMRI detected wall motion abnormality; no abnormality was found on T2WI, T1WI. Conclusion The part ligation approach to make rabbit hibernating myoeardium model is feasibility, and CMRI is a kind of promising approach for evaluating HM.
