应用SPECT探索抑郁症局部脑血流变化规律 被引量:7

摘要 目的探讨抑郁症患者和正常对照组在认知与基础状态下局部脑血流(rCBF)变化规律。方法选取9例确定为抑郁症患者和9例年龄相匹配的健康被试为对照组,利用隔日显像法,分别对两组被试在认知和基础状态下进行SPECT脑血流灌注显像。利用StatisticalParametricMapping(SPM2)软件分析。结果认知相对于基础状态:①正常被试额上回、前额右侧和BA(Brodmannarea)6脑血流显著升高;颞叶、颞上回区血流降低。②抑郁症患者右扣带回和尾状核左侧脑血流显著升高;海马旁回右侧、中央后回血流显著降低。结论两组被试在基础和认知状态下的SPECT脑激活,在抑郁症治疗诊断中具有重要的临床参考价值。 Objective To probe into the variation of regional cerebral blood flow (rCBF) of the depressed patients as well as the normal control group in the basic and cognitive state. Methods SPECT imaging was performed in 9 patients with the depression disorder diagnosed with ICD-10 or BECK, and 9 cases of age matched normal controls for comparison. All sub jects performed cognitive task in the first day. The third day is basic. Situation of rCBF in depression and control group were analyzed by SPM2 software. Results ①Compared with the basic state, in the cognitive state the blood flow of normal subjects in superior frontal gyrus, right frontal lobe and BA 6 were significantly increased, and the rCBF of temporal, superior temporal gyrus were significantly decreased. ②Compared with the basic state, in the cognitive state the blood flow of callosal gyrus and putamen were increased, and parahippocampal and posterior cingulate were decreased in depression. Conclusion SPM analysis showed that significant rCBF change in normal and depression related to clinical diagnosis.
