摘要
目的探讨肿瘤标志物谷氨酰转肽酶(GGT)、a-L岩藻糖苷酶(AFU)和甲胎蛋白(AFP)在原发性肝癌(PHC)诊断中的价值。方法对PHC组84例、转移性肝癌(MC)组48例、肝炎组57例和正常对照组79例,检测血清GGT、AFU和AFP水平(前三组患者在接受治疗前检测)。结果PHC组血清GGT、AFU和AFP水平均明显高于肝炎组及正常对照组,差异有非常显著性(P<0.01)。PHC组血清GGT、AFU和AFP阳性率分别为91.7%、80.9%、69.0%,AFU、AFP联检阳性率92.4%,GGT与AFU、AFP联检阳性率97.1%;AFP阴性的PHC26例中,血清GGT、AFU阳性率分别为92.3%(24/26)、76.9%(20/26),表明二者对AFP阴性的PHC患者具有较高的诊断互补作用。结论血清GGT、AFU和AFP联合检测可提高PHC的诊断率,特别是对AFP阴性、低浓度的PHC更具有诊断价值。
Objective To investigate the clinical value of tumor marker r - glutamyle transpeptidase (GGT), alpha- fucosidase (AFU) and alphafetoprotein(AFP) in the diagnose of PHC. Methods AFP, AFu, GGT in the serum were studied in 84 patients with PHC, 48 with hepatic metastases, 57 with hepatitis, and 79 healthy individuals . Results The serum GGT,AFU and AFP in PHC were significantly higher than that in the hepatitis and healthy individuals( P 〈 0.05 or P 〈 0.01 ). The positive detection rate of GGT, AFU and AFP in PHC was 91.7% , 80.9% and 69.0% respectively and combined assay of AFU and AFP 92.4%. combined assay of GGT,AFU and AFP will increase the detecting rate up to 97.1%. In 26 cases with AFP - negative PHC, the positive detection rate of GGT, AFU was 92.3% ,76.9% respectively, which showed its complementary function . Conclusion The combining GGT, AFU and AFP examination are superior to any single marker examination and can greatly increase the detecting rate of PHC and was valuable especially in those patients with negative or low serum level of AFP.
