胚胎质量对冻融胚胎移植结局的影响 被引量:1

摘要 目的探讨移植胚胎数量和质量与对冻融胚胎移植结局的影响。方法回顾性分析2004年间用慢冻快融的方法所进行的265个冻融胚胎移植周期结果,并与同期新鲜胚胎移植周期比较两者的临床妊娠率、种植率、多胎率等。结果:(1)移植3个胚胎组的临床妊娠率、多胎率、种植率分别为35.19%、36.84%、16.67%,与移植≤2个胚胎组的24.49%、1667%、15.48%的差异均无显著性(P>0.05)。移植3个优质胚胎组的多胎率为40.82%明显高于移植≤2个优质胚胎组的2308%(P<0.01),但两者的临床妊娠率和种植率均无显著性差异(P>0.05)。(2)妊娠组胚胎完好率、优质胚胎率分别为4697%,68.18%显著高于非妊娠组的40.65%,55.91%(P<0.05)。而两者的复苏率分别为84.24%、79.09%,无显著性差异(P>005)。(3)与同期新鲜胚胎移植周期的种植率27.62%比较,冻融胚胎移植的种植率13.28%显著降低(P<0.01)。结论冻融胚胎移植周期的多胎妊娠率与移植的优质胚胎数目有关,与移植胚胎数目无关。冷冻胚胎移植的种植率显著低于新鲜胚胎移植,两者的临床妊娠率及多胎率均无差别。 Objective To evaluate the influence between the embryos' quantity and quality on the pregnancy outcome of frozen-thawed embryo transfer (FET). Methods A retrospective study of 265 FET cycles and 400 fresh embryo transfer cycles were analyzed in 2004 and compared the clinical pregnancy rates, implantation rates, multifetal pregnancies rates between two groups. Results (1) There is no significant difference in the clinical pregnancy rates when 3 or≤2 embryos were transferred. The multifetal pregnancies rates of 3 good quality embryos were transferred is significant higher than the rates of ≤2 good quality embryos were transferred (40.82% Vs. 23.08% respectively P〈0.01). There are no significant difference between these two groups in the clinical pregnancy rates implantation rates (P〉0.05). (2) The percentage of 100%-intact embryos and good quality embryo were higher in pregnancy cycle than non- pregnancy cycle (46.97% Vs 40.65%, 68.18% Vs 55.91%, respectively P〈0. 05). (3) The implantation rates of frozen-thawed embryo transfer cycle was significantly lower than fresh embryo transfer cycle(13.28% Vs.27.62% respectively P〈0. 01). Conclusions Our data suggested that there is a relationship between the clinical pregnancy rates and the number of good- quality embryos transferred of FET. There are no significant difference between quantity of embryo were transferred. The implantation rates of frozen-thawed embryo transfer cycle was significantly lower than fresh embryo transfer cycle. There are no significant difference between frozen-thawed embryo transfer cycle and fresh embryo transfer cycle in clinical pregnancy rates and multifetal pregnancies rates.
