肩周封闭结合康复训练治疗肩手综合征疗效观察

The effect of blocking around shoulder and rehabilitation training on treating shoulder-hand syndrome

目的探讨肩周封闭疗法配合康复训练治疗治疗脑卒中后肩-手综合征的疗效。方法60例脑卒中后肩-手综合征患者随机分为两组,单纯治疗组采用康复训练疗法,复合治疗组在康复训练疗法的基础上加用肩周封闭治疗,于治疗后4周对患者肩关节疼痛程度(采用视觉模拟评分即VAS法)、肩关节活动度、上肢运动功能(简化Fugl-Meyer)进行评定。结果复合治疗组的疼痛评分明显低于单纯治疗组(P<0.05),肩关节活动度(外展、外展上举)明显>单纯治疗组(均P<0.05或0.01),Fugl-Meyer运动功能评分明显高于单纯治疗组(P<0.05)。结论肩周封闭配合康复训练治疗脑卒中后肩-手综合征疗效显著。

Objective To study the effect of blocking around shoulder binding rehabilitation training on treating shoulder-hand syndrome caused by acute cerebrovascular disease. Methods Sixty patients with shoulder-hand syndrome caused by acute cerebrovascular disease were divided into two groups. Rehabilitation training was applied to pure therapeutic group. Compound group was added by blocking therapeutics around shoulder besides rehabilitation training. Pain score of shoulder joint（VAS）, activity of shoulder joint involving abduction and abduction raising and the simplified Fugl-Meyer grade of upper limb movement were evaluated after four weeks from the initial treatment. Results The pain score of compound group was obviously lower than that of pure therapeutic group （P 〈0.05）. Activity of shoulder joint involving abduction and abducting raising and the Fugl--Meyer grade of compound group were obviously higher that of pure therapeutic group （P〈 0. 05,0. 01）. Conclusions Blocking around shoulder with rehabilitation training has significantly therapeutic effect on shoulder-hand syndrome after acute cerebral accident.
