耻骨上前列腺切除术后不同年龄组患者控尿及生存质量的1年随访

Urine Control and Quality of Life in Patients after Suprapubic Prostatectomy of Different Ages: one-year Follow-up

目的:客观了解不同年龄段耻骨上前列腺切除术后患者控尿恢复情况及生存质量的差异。方法:按自行设定标准纳入观察的良性前列腺增生症患者86例,依据年龄不同分为≤60岁(A组)、61-70岁(B组)及≥71岁(C组),各30、28及28例,均行耻骨上前列腺切除术。术前均进行国际前列腺症状评分(IPSS)和生存质量(QOL)评定;术后第1、3个月及1年时随访,观察控尿恢复时间及QOL。结果:控尿恢复率,在术后第1、3个月时,A组明显优于C组(P<0.01和0.05);B组与C组比较差异无显著性意义;6个月、1年时3组均差异无显著性意义。IPSS及QOL评定,术后3个月,C组明显高于A、B组;最大尿流率(Qmax)评分低于A、B组(P<0.05);IPSS及剩余尿量(PVR)亦显著高于A、B组(P<0.01)。6个月时3组间各项指标均差异无显著性意义。结论:不同年龄段患者行耻骨上前列腺切除术后的控尿恢复及生存质量亦不同。≥70岁患者术后恢复控尿相对延迟,致生存质量受到影响。

Objective： To investigate the difference of the recovery of urine control and quality of life （QOL） in patients after suprapubic prostatectomy of different ages. Methods： Eighty-six patients with benign prostatic hyperplasia were observed according to the self-designed inclusive criteria. According to age, they were divided into ≤60 years old group （A, n=30）, 61-70 years old group （B, n=28） and ≥71 years old group （C, n=28）, and subjected to suprapubic prostatectomy. The symptoms and QOL of the patients were assessed before operation, and the patients were followed up at the 1st and 3rd month and 1st year after operation regularly. The recovery time of urine control and QOL of the patients were investigated. Results： There was significant difference in the recovery rate of urine control on the postoperative month 1 and 3 among three groups （P〈0.01）, significantly higher in group A than in group C （P〈0.01, 0.05）. On the 6th month and 1st year postoperation, there was no significant difference in the recovery rate of urine control among three groups. On the postoperative month 3, QOL scores were significantly higher and Qmax lower in group C than in groups A and B （P〈0.05）, and IPSS and PVR in group C were also significantly higher than in groups A and B. On the 6th month postoperation, there was no significant difference in IPSS, QOL, Qmax and PVR scores among the three groups. Conclusion： The recovery of urine control and QOL are different in patients at different ages after suprapubic prostatectomy. The postoperative recovery of urine control is relatively delayed in the patients above 70 years old.
