肩胛骨松解法治疗慢性肩周炎的康复疗效

Scapula Dissolution in the Treatment of Chronic Periarthritis of Shoulder

目的:探讨肩胛骨松解法结合关节松动术在治疗慢性肩周炎中的康复疗效。方法:对已确诊的慢性肩周炎患者60例随机分为A、B 2组各30例,均接受关节松动术治疗方案,A组并增加肩胛骨松解法,治疗前后比较2组患者肩关节疼痛程度及ROM改善情况。结果:治疗20 d后,疼痛视觉模拟评分2组与治疗前比较均显著降低(P<0.01),2组间比较,A组下降更明显(P<0.01);ROM评定,2组肩关节角度均有较大提高,但A组肩关节前屈、后伸、外旋及外展、内旋的活动范围均明显大于B组(P<0.01);复发率比较,2组间差异无显著性意义。结论:肩胛骨松解法结合关节松动术治疗慢性肩周炎效果显著,尤其在疼痛的改善、肩关节活动范围的提高方面明显优于单纯关节松动术。

Objective：To study the effectiveness of scapula dissolution combined with the rhythm mobilization of joint treating chronic periarthritis of shoulder. Methods：Sixty patients diagnosed having chronic periarthritis of shoulder were treated with the rhythm mobilization of joint and divided randomly into groups A and group B （30 cases in each group）. The scapula dissolution was done in group A. Before and after treatment, the pain and ROM of shoulder joint were compared between two groups. Results：After treatment for 20 days, VAS scores was decreased significantly as compared with those before treatment （P〈0.01）, but more significantly in group A than in group B （P〈0.01）. After treatment, ROM was increased significantly as compared with that before treatment （P〈0.01）. But flexion, extension, external rotation, abduction and internal rotation of shoulder joint in the group A were greater than in the group B （P〈 0.01）. There was no significant difference in the recurrent rate between two groups （3.3 % vs 6.9 %）. Conclusion： Treatment of chronic periarthritis of shoulder with scapula dissolution and rhythm mobilization of joint has excellent clinical outcome, and better curative effect than single rhythm mobilization of joint especially in releasing pain and increasing the ROM of flexion, extension, abduction of shoulder joint.
