食管癌术后乳糜胸的临床分析

Clinical analysis of chylothorax following esophagectomy

目的探讨食管癌术后乳糜胸的防治方法。方法回顾性分析14例食管癌术后并发乳糜胸患者的临床资料。结果1362例食管癌术后发生乳糜胸14例,发生率1.03%(14/1362),非手术治疗5例,手术治疗9例,均痊愈出院。结论食管癌术后乳糜胸应本着预防为主、综合治疗的原则,膈上低位胸导管结扎术对治疗和预防食管癌术后乳糜胸,疗效确切。

[Objective] To investigate the prevention and treatments for chylothorax following esophagectomy. [Methods] Restropective analysis was given to the clinical data of 14 cases with chylothorax following esophagectomy. [Results] Chylothorax occurred in 14 cases of 1362 patients with post-esophagectomy, the incidence was 1.03%（14/1362）. Of them, 5 cases received non-surgical management and the remaining 9 cases were subjected to surgical treatments, all patients were cured. [Conclusions] The therapeutic principles of the chylothorax following esophagectomy are prevention and synthetic treatments. The efficacy of ligation of the thoracic duet above the diaphrenic level for treatment prevention of postoperative chylothorax is definitive and accurate,
