瑞芬太尼复合异丙酚或地氟醚用于支撑喉镜手术麻醉恢复期的对比观察

Comparison of remifentanil plus propofol with remifentanil plus desflurane on recovery from aneshesia for suspension laryngoscopy

目的对比观察瑞芬太尼复合异丙酚或地氟醚对支撑喉镜手术麻醉恢复期的影响。方法ASAⅠ～Ⅱ级择期拟行支撑喉镜手术的患者60例,随机分成P和D组(n=30),两组均以瑞芬太尼复合异丙酚(7～8mg·kg-1·h-1)(P组)或地氟醚(呼气末浓度6%)(D组)维持麻醉。术毕记录麻醉恢复时间和术后并发症。结果D组的麻醉恢复时间短于P组(P<0.05),但差异幅度均小于5分钟;两组术后并发症无明显差异(P>0.05)。结论瑞芬太尼复合异丙酚或地氟醚用于支撑喉镜手术均能达到快速、安全地苏醒,而地氟醚组苏醒更快些。

Objective To compare the recovery after anesthesia with a combination of remifentanil with propofol or desflurane during operation with suspension laryngoscopy. Methods Sixty ASA Ⅰ-Ⅱ patients scheduled for the surgery through suspension laryngoscopy were randomly divided into 2 groups （n=30）. Anesthesia induction was identical in both groups followed by a combination of remifentanil with either desflurane （6.0 vol%）（D group） or propofol （7-8 mg·kg-1·h^-1） （P group）for maintenance.During recovery period, recovery times were recorded at defined points. Postoperative complications were also recorded. Results Recovery times were significantly shorter in group D than in group P, but the extent of these difference was small（〈5min）; there was no significant difference in the incidences of postoperative complications in both groups. Conclusion Remifentanil combined with propofol or desflurane, used for operation with suspension laryngoscopy, will facilitate a rapid and safe emergence. Recovery times after remifentanil-based anaesthesia are slightly shorter with desflurane than with propofol.
