摘要 经六年大量临床验证,光动力疗法已成为具有高度选择性和稳定可靠疗效的无瘢痕治疗鲜红斑痣的新方法。为解决治疗后的较长时间皮肤光敏反应这一主要副作用,本文对21例志愿鲜红斑痣患者的26个病灶应用血啉甲醚(HMME)为光敏剂进行了治疗,对HMME的作用进行了临床观察,发现HMME具有良好的疗效、较轻的局部反应、较短的避光期和可行重复治疗的间隔期等优点,值得对其进行深入的研究。 Photodynamic therapy for port wine stains (PWS) has been used clinically since 1991, and proved to be a highly selective, specific and no scarring new modality for treatment of this disease. However, the main side effect of PDT is long term skin photosensitivity after the treatment because of the slow elimination of hematoporphyrin derivative(HpD) from the skin. For solving this problem, 21 PWS volunteers accepted a new photosensitizer hematoporphyrin monomethyl ether(HMME) in the treatment. 26 lesions were treated in total with HMME i.v. in a dose of 4 5-6 0 mg/kg. Three to six months follow up after one treatment showed that 4 lesions had excellent result, 14 had good result, 8 got fair result and no one case showed scar formation. The skin photosensitivity disappeared completely within 3 weeks in all 21 patients, and 16 of them got it within 2 weeks. This preliminary clinical study showed that HMME can be eliminated from the skin much faster than HpD, and might be a better photosensitizer in PDT for PWS.
