B超预测腹腔镜胆囊切除术的价值

Valuee of B-mode Ultrasound Examination in Estimating Degree of Difficulty of Laparoscopic Cholecystectomy

通过８４例腹腔镜胆囊切除术术前Ｂ超与手术结果的对照，分析了在胆囊、胆囊壁、胆周和胆囊结石等方面Ｂ超声像图不同的病例的手术情况，阐明了Ｂ超预测腹腔镜胆囊切除术的作用，对于降低手术中转率，防止和减少手术并发症具有重要临床价值。应该指出，Ｂ超声像图显示胆囊颈部结石嵌顿伴胆囊增大或胆囊壁增厚、胆囊增大伴胆囊壁增厚的病例，腹腔镜胆囊切除术要慎重。

cases of B-mode ultrasonograph obtained before laparoscopic cholecystectomy were reviewed with the postoperative result.Different images of gallbladder, gallbladder wall, gallbladder surrounding and gallstone were analized with reference to the difficulties in the procedure.This evaluation might have clinical significance in reducing rate of reversion of operation and prevention or reduction of operative complications. It was suggested that when B-mode ultrasonographic images showing gallbladder neck with incacerated stone and enlarged gallbladder, or thicked gallbladder wall, and enlarged gallbladder with wall thickening,the laparoscopic cholecystectomy must be performed cautiously.
