高频超声动态诊断小儿先天性肥厚性幽门狭窄

摘要 目的探讨高频超声动态诊断先天性肥厚性幽门狭窄(CHPS)的价值。方法在15例疑为CHPS的患儿胃管内注入液体,经高频超声动态观察液体通过幽门管时的变化并测量幽门肌层厚度、幽门直径及幽门管长度。结果15例患者中,超声诊断14例与手术结果符合,符合率93%。1例为肠旋转不良。结论高频超声动态诊断基本上可代替X线碘油造影或X线钡餐(GI)检查,可作为首选诊断方法。 Objective To investigate the value of high frequency uhrasounds dynamic diagnosis of congenital hypertrophic pyloric stenosis (CHPS). Methods 15 cases suspicious of CHPS sick children patients were injected fluid into stomach, through high frequency ultrasound dynamic observation to match variation when fluid pass through pyloric canal and check the thickness of pyloric muscular thickness, diameter of pylorus and length of pyloric canal. Results Among 15 cases, 14 cases of ultrasound diagnosis were the same as the operative finding. Agreement rate reach 93%, one case was intestinal incomplete rotation, Conclusion High frequency ultrasound dynamic diagnosis basically can use instead of X ray iodine photography, or X way barium meal (GI) examination, and which could serve as the first choice of diagnostic method.
