目的:研究赤道劈裂法超声乳化对眼前段结构的影响,超声乳化时间及术后效果对比分析。方法:2002-10/2004-10我中心住院白内障部分患者232例256眼,男112眼,女144眼,随机分成两组,赤道劈裂法超声乳化组(实验组)134眼,刻核法超声乳化组(对照组)122眼,观察超声乳化有效时间,后囊破裂情况,角膜内皮、眼压、视力等指标。结果:超声乳化有效时间:实验组平均105s,平均能量10.53%。对照组平均223s,平均能量18.5%。二者有明显差异(P<0.01)。后囊破裂:实验组4例,占3.0%,对照组7例,占5.7%。内皮细胞计数平均丢失率实验组6%,对照组4%。眼内压:实验组术前平均眼压16.23mmHg,术后平均下降2.6mmHg。对照组术前平均眼压16.83mmHg,术后平均下降2.0mmHg.视力:劈裂法术后矫正视力0.5以上117眼,占87.3%;刻核法术后矫正视力0.5以上95眼,占77.9%结论:劈裂法超声乳化具有明显缩短白内障超声乳化时间,降低乳化能量,减少后囊破裂、虹膜及角膜内皮损伤、保护视功能等作用,是中等以上硬度晶状体核超声乳化理想的手术方法。
AIM: To study the effect of phacoemulsification using e- quator chop technique in the papillary plane on ocular anterior segment and visual function.
METHODS: A total of 232 senile cataract patients (256 eyes) were included and randomly divided into two groups: experiment group (134 eyes) using equator chop technique and control group (122 eyes) using sculpture technique. The effective phaco time, average phaco energy, posterior capsular rupture, endothelial density, intraocular pressure (IOP) and visual acuity were observed.
RESULTS: The effective phaco time, average phaco energy, posterior capsular rupture and endothelial cell loss rate were 1'45", 10.53%, 4 cases (3%) and 6% respectively, compared to 3'43", 18.5%, 7 cases (7.3%) and 4% in the control group. IOP was decreased by 2.6mmHg in the experimental group and 2.0mmHg in the control group. The postoperative corrected visual acuity was ≥0.5 in 117 cases (87.3%) of the experimental group, whereas in 95 cases (77.9%) of the control group.
CONCLUSION: Phacoemulsification using lens equator chop can definitely reduce the phaco time and average phaco energy, decrease complications such as corneal endothelial cell damage and posterior capsular rupture. It has great advantages to medium-hardness nuclear cataract phacoemulsification.
