全反式维甲酸抑制兔晶体后囊膜混浊实验观察

摘要 目的研究全反式维甲酸对兔晶状体后囊膜混浊的抑制作用及有效抑制浓度。方法新西兰大白兔12只24只眼,分为右眼实验组(A组)和左眼对照组(B组),各12只眼。A组兔眼行白内障囊外摘除术(ECCE术),前房灌注平衡盐液(BSS)中加入5.63μg/ml或6.06μg/ml的全反式维甲酸,B组兔眼行ECCE术,BSS中不加入维甲酸。术后1月至术后4-4.5月观察结膜、角膜、前房炎症及后囊膜情况。术后4~4.5月取出兔眼标本,石腊包埋及切片后病检。结果术后第1~4天术眼有不同程度的结膜浅充血、睫状充血、角膜水肿、房水混浊,术后第5天角膜水肿、房水混浊明显好转,1周后结膜浅充血及睫状体充血消退,两组差异无显著性意义(P〉0.05);B组兔晶状体后囊膜梭形细胞及纤维细胞增生、后囊膜增厚明显高于A组(P〈0.005)。结论全反式维甲酸浓度为5.63μg/ml及6.06μg/ml对兔晶状体后囊膜混浊有抑制作用。 Objective To investigate the concentration-dependent inhibitory effect of all-trans retinoic acid (RA) in preventing posterior capsule opacification (PCO) in rabbit lens epithelial cells (RLEC) , and give a clue to the study on the prevention of PCO. Methods Twenty-four eyes from twelve rabbits were divided into two groups. Group A ( 12 right eyes ) was undertaken ECCE surgeries using BSS plus RA (5.63 or 6.06 μg/ml) , while group B ( 12 left eyes) had ECCE surgeries with BSS only and served as control, Slitlamp, ophthalmoloscopy and anterior segments were monitored for 4 to 4, 5 months following operation. Furthermore, proliferation of lens epithelial cells was evaluated histologically 4 to 4.5 months after surgery, Results Clinical assessments showed that RA treatment during surgery was very effective in preventing PCO compared with the untreated controis ( P 〈 0.005 ). Histological analysis also revealed a significant reduction in cell proliferative activity on posterior capsules in the RA-treated group in contrast to the appearance of muhilayered ceils in the control group, There was no obvious difference in cornea, anterior chamber, iris, ciliary body and cytological analysis of aqueous humor between two groups. Conclusion RA significantly prevents the development of PCO in rabbit eyes without obvious toxicity to the normal ceils in the anterior segment, Optimal biocompatible dosage must be carefully determined by further investigation.
