三尖瓣发育不良综合征的超声诊断初步探索

Preliminary Investigation of Diagnostic Value of Echocardigraphy in Hypoplastic Tricuspid Valve Syndrome(HTVS)

目的:探讨三尖瓣发育不良综合征的超声图像特点,提高诊断的正确率。方法:对8例先天性三尖瓣发育不良综合征的类型及超声图像特点进行回顾性分析,并与手术结果和心导管检查相对比。结果:①3例病人与手术结果完全一致,3例主要诊断乎后手术结果,2例误诊。②三尖瓣发育不良综合征为多种疾病共有,本组以三尖瓣(TV)狭窄伴肺动脉瓣(PV)狭窄最多(4例)。(PV)闭锁伴间隔完整2例,三尖瓣闭锁2例。③并发畸形多种多样,有房间隔缺损(ASD)8例,室间隔缺损(VSD)3例,动脉导管未闭(PDV)5例,体静脉或肺静脉畸形引流4例,大血管转位2例。结论:三尖瓣发育不良综合征畸形复杂,要多节段的全面超声检查分析,必要时结合声学造影和心导管检查才能得出全面正确的诊断。

Objective.To explore the imaging charcteristic of echocardiography on HTVS and to improve the accuracy rate of diagnosis by Echd. Methods： Eight cases of HTVS were studied. Their types and characteristics of Echo were analyzed retrospectively. The results of ultrasounic examination were compared with surgical findings and angiocardiography. Results. ①Three cases were accurately diagnosed by Echo, The main diagnosis of other htree cases were in accordance with surgical findings. Misdiagnosed by Echo accurred in two cases of them.②HTVS was caomposed of many kinds of malformation, such as thicuspid stenosis with pulmonary stenosis （4 ca ses）. Pulmonary stresia with intact ventricular septum（2 cases） and tricuspid artesia （2 cases）.③HTVS was complexed by deformity of many kinds. such as ASD （8 cases）, VSD （3 cases）, PDA（5 cases）, anomalies of pulmonary vein and systemic venous system（4 cases）, transposition of great arteries（2 cases）. Conclusion： HTVS is a complicated malformation and the imaging characteristics of Echo is small right ventricle and tricupid ring or tricuspid atresia. We should combine contrast Echo with angiocardiography to get accuracy diagnosis.
