摘要
目的探讨磁共振灌注成像评价moyamoya病的脑组织血供情况的作用。方法回顾性分析25例moyamoya病患者的临床及影像学资料,对所有患者均行常规MR检查及灌注成像检查。分析病变脑组织的血流灌注状况。结果MRI平扫有14例脑组织信号表现为正常。MRA显示所有病例双侧大脑中动脉及远端分支狭窄或显影中断;22例显示有侧支血管网形成,其中15例表现为双侧对称,7例见两侧血管网数量不对称。脑灌注成像(PWI)显示平均通过时间(mean transit time,MTT)延长0.5~15S。局部脑血流量(regional cerebral blood flow,rCBF)下降23例,下降幅度为26%~56.5%。局部脑血容量(regional cerebral bloodv olume,rCBV)有9例正常,16例下降。结论MR脑灌注成像可以有效地评价moyamoya病的脑组织的血供。
Objective to evaluation the blood supply of brain tissue of moyamoya disease with MR perfusion-weighted imaging(PWI). Methods The clinical and imaging data of 25 patients with moyamoya disease were analyzed retrospectively. All of the cases were performed with convention and perfusion-weighted MR imaging. The brain tissue flood perfusion were analyzed. Results Fourteen cases were normal in convention MR imaging, but bilateral middle cerebral artery (MCA) and/or distal artery of MCA were stegnosis or occlusion in all of the cases. Collateral vasoganglion were displayed in 22 cases, there were symmetry in 15 cases and were dissymmetry in other cases. Perfusion-weighted imaging revealed MTT(mean transit time) was extend 0.5s to 15s. 23 cases showed decreased in rCBF (regional cerebral blood flow), the amplitude was 26% to 56.5%. There were 9 cases of normal and 16 with decreased in rCBV (regional cerebral blood volume). Conclusion MR perfusion-weighted imaging could appraise blood supply of brain tissue with moyamoya disease efficaciously.
