枕叶梗死与头痛

OCCIPITAL LOBE INFARCTION AND HEADACHE

目的:分析枕叶梗死与头痛的关系,探讨脑梗死头痛的发病机制、脑梗死头痛与偏头痛的关系以及枕叶在这两者中的作用。方法:回顾性研究125例枕叶梗死患者及152例非枕叶梗死患者头痛发生率及头痛特征。结果:277例脑梗死头痛发生率19.49%;枕叶梗死头痛发生率26.4%,明显高于非枕叶梗死头痛发生率(P<0.01);椎-基底动脉系统梗死的头痛发生率明显高于颈内动脉系统梗死的头痛发生率(P<0.01),且以枕叶、脑干尤为明显。结论:脑梗死与偏头痛关系密切,偏头痛是亚临床的后循环梗死的独立危险因素。枕叶在脑梗死头痛及偏头痛发作中可能起到相似的作用。

Objective： To explore the relationship between headache attack and occipital lobe infarction, the pathogenesy of headache in cerebral infarction, the relationship between headache of cerebral infarction and migraine, and the role of occipital lobe in headache and migraine. Methods： A retrospective study was made on the incidence rate and characteristics of headache in 125 occipital infarction cases and 152 non-occipital lobe infarction cases. Results： Headache incidence rate was 19.49% in 227 cerebral infarction cases. Headache incidence rate （26.4%） in occipital lobe infarction cases was higher than that in non-occipital lobe infarction （P 〈 0.01 ）. Headache incidence rate in vertebra-basilar artery system infarction was significantly higher than that in carotid artery system infarction （P 〈0.01 ）, especially in occipital lobe and brainstem. Conclusion： Cerebral infarction has close association with migraine. Migraine is an independent risk factor of subclinical post-circulate infarction. Occipital lobe probably plays a role in cerebral infarction headache similar to migraine.
