对三套不同图形探索性眼球活动试验的评价

The three sets of figures as stimulus for exploratory eye movement test in schizophrenia

目的验证应用自编的两套图形所进行的探索性眼球活动试验对于精神分裂症的诊断价值,并与日本Kojima等创立的S图形作比较。方法对231例精神分裂症患者和274名正常对照者(正常对照组),在同一天内用三套不同图形进行探索性眼球活动检测,测定各图的凝视点数(NEF)和反应性探索评分(RSS),并计算其敏感度、特异度和判别值(D);分析三套图形各项数据的相关性。结果(1)在三套图形进行眼动试验中,精神分裂症组的NEF、RSS和D均低于正常对照组(P<0.001)。(2)三套眼动试验的敏感度和特异度均相似。(3)三套眼动试验的NEF、RSS和D值有很好的相关性(均P<0.001)。结论用自行编制的两套图形所做的眼动试验,对精神分裂症的诊断价值与Kojima等所用的S图形相似,三套图形可以相互替代使用。

Objective To validate two new self-designed sets of figures in comparison with the original Kojima＇s S-shape figure for exploratory eye movement in schizophrenia. Methods The participants were 231 patients with schizophrenia and 274 normal controls, and were instructed to have the exploratory eye movement test with the original S-shape as stimulus firstly, then with the two new sets of figures as stimulus on the same day. The number of eye fixation （NEF） and the responsive search score （RSS） in each test were measured, and the sensitivity and the specificity were calculated. The discrimination function was used to calculate the D score. Correlation analysis was performed to obtain the correlation coefficient （γ） among three sets of figures. Results Regardless of the figure stimulus, NEF, RSS and D in patients were significantly lower than that in controls （ P 〈 0. 001 ）, The two new sets of figure had similar sensitivity and specificity to the original S-shape figure. The sensitivity and specificity of NEF2 were 62% and 62% respectively, and those of RSS2 were 71% and 58% respectively, while those of D2 were 63% and 66% respectively. NEF, RSS and D in 3 sets were correlated with each other （ r = 0. 405-0. 620, P 〈 0. 001 ）. The discrimination function could differentiate patients with schizophrenia from normal controls, Conclusion The three sets of figures as stimulus can be replaced each other in measuring the exploratory eye movement impairment in schizophrenia.
