临床病理学参数对大肠癌淋巴结转移的影响

Effect of Clinicopathological Parameters on Lymphatic Metastasis of Colorectal Carcinoma

目的探索影响大肠癌淋巴结转移的临床病理参数,发现淋巴结转移的高危人群。方法对1990年1月至1999年12月间施行根治性切除的1051例大肠癌进行回顾性分析,研究临床病理学参数对淋巴结转移的影响。结果影响淋巴结转移的因子包括年龄、有症状、有家族史、肿瘤部位、T分期、病理分级、大体类型、组织类型。逻辑回归分析显示影响淋巴结转移的强度依次为部位>家族史>浸润深度>年龄>病理分级,症状、大体类型、组织类型则影响不大。结论利用临床病理学参数可以预测和发现病理检查无淋巴结转移但有转移倾向的高危人群。

Objective To investigate the clinicopathological parameters affecting the lymphatic metastasis （LM） of colorectal carcinoma （CRC）, and discovering the patients with high risk of LM. Methods 1 051 CRC cases undergone radical resection between January, 1990 and December, 1999 were retrospectively investigated in the effect of clinicopathological parameters on LM. Results The clinicopathological parameters affecting LM include age, positive symptom, family history, tumor site, T staging, cellular differentiation, gross classification, histological type. Logical regression analysis showed that the strength of affecting LM was decreasing in order of tumor site, family history, invasive depth, age and cellular differentiation. Condusion CRC patients without LM found by pathological test, but in tendency to LM can be predicted and discovered by clinicopathological parameters.
