摘要
目的探讨血清中氨基酸末端脑钠素前体(NT-proBNP)水平与心房颤动(房颤)之间的关系。方法病例组心血管病患者136例,心功能Ⅰ-Ⅱ级(NYHA分级),其中房颤患者90例,窦性心律患者46例;健康对照组30例。应用竞争性酶联免疫分析(ELISA)定量测定血清中NT-proBNP水平,并进行统计学分析。结果房颤患者血清NT-proBNP水平显著高于窦性心律患者和对照组(P< 0.01),窦性心律患者与对照组之间血清NT-proBNP水平差异无统计学意义(P>0.05);房颤、年龄、右心房内径是血清NT-proBNP水平升高的独立预测因素。结论房颤患者血清NT-proBNP水平明显升高,监测血清NT-proBNP水平的变化有助于房颤患者心功能的评价。
Objective To investigate the relationship between serum N-terminal fragment pro-brain natriuretic peptide(NT-proBNP) levels and atrial fibrillation (AF). Methods Serum NT-proBNP levels were measured by ELISA in 136 subjects(90 with AF, 46 in sinus rhythm). Subjects were excluded if they had a history of myocardial infarction, cardiomyopathy, rheumatic heart disease, or hyperthyroidism that proceded the onset of AF. Control subjects(30) were obtained from a healthy outpatient primary care population. Potential determinants of serum NT-proBNP levels were identified by univariate and multivariate. Results Individuals with AF had higher serum NT-pro BNP levels than those with sinus rhythm and control subjects( P 〈 0. 01, respectively). In individuals with sinus rhythm and control subjects did not show difference at serum NT-proBNP levels ( P 〉 0. 05 ). The final regression model showed that presence of AF, older age and larger right atrial diameter were independent predictors of higher serum NT-proBNP values. Conclusions Presence of AF in patients is associated with elevated serum NT-proBNP levels. Examining the change of serum NT-proBNP levels has evaluation value in cardiac function of patients with AF.
