氯诺昔康术后镇痛对血液流变学的影响 被引量:1

摘要 目的观察下肢骨折术后持续输注氯诺昔康镇痛对血液流变学的影响。方法90例下肢手术病人随机分成L组(氯诺昔康组)、F组(芬太尼组)和K组(对照组),每组30例。L组氯诺昔康负荷量为8mg,术后持续输注0.064%氯诺昔康48h;F组芬太尼负荷量为0.1mg,术后持续输注0.001%芬太尼48h。K组不使用镇痛。分别于术前(T0)、术后24h(T1)、48h(T2)采空腹静脉血测定血液流变学各项指标。结果全血粘度L组术后(T1、T2)较术前(T0)有明显下降(P<0.05),L组与F组及K组比较也有明显下降(P<0.05),F组比K组稍有下降(P>0.05);血浆粘度术后K组较L组及F组明显升高(P<0.05)。结论术后持续输注氯诺昔康镇痛的同时也降低了患者的血液流变学指标,有利于改善患者术后血液的高粘状态。 Objective To investigate the effects of lomoxicam infused continuously for analgesia (PCIA) on hemorheology in the patients underwent lower limb operations. Methods Ninty patients were randomly divided into group L (lornoxiearn group), group F (fentanyl group) and group K (blank control group). The patients in group Lwere given loading dose lornoxican 8 nag followed by 0. 064% lornoxicam infusion for 48 h, those in group F loading dose 0. 1 nag fentanyl followed by 0. 001% fentanyl for 48 h, and those in group K were not given PCIA. Fasting blood samples were taken from peripheral vein before opration (T 0),at 24 h (T 1)and 48 h (T 2) after operation for the hemorheologic analysis. Results Blood viscosity of group L at T land T 2 was obviousiy lower than that of T 0 and that of group F and group K (P〈0.05). Plasma viscosity in group K was obviousiy higher than that in group L and group F. Conclusion PCIA with lornoxicam may reduce the parameters of hemorheology and improve the high viscosity of blood during postoperative analgesia.
