摘要 目的:总结并分析运动神经元病的发病机制的相关学说,综合探讨运动神经元病的发病机制,从而为临床治疗上提供可靠的依据。资料来源:应用计算机检索PUBMED1994-01/2001-12关于运动神经元病的文章。检索词“neurodegenerativediseases”,并限定文章的语种类为英文。资料选择:对资料进行初审,纳入标准:关于运动神经元病的发病机制。排除标准:重复性研究。资料提炼:共收集到符合上述要求的文献43篇,排除24篇重复性研究。19篇符合纳入标准。资料综合:运动神经元病的发病机制的相关学说主要包括自由基氧化、兴奋性毒性、遗传、免疫因素,神经营养因子的缺乏和神经微丝的损伤等,不同的发病机制假说都有各自的证据和研究进展。结论:把不同的机制假说孤立起来单独研究,不利于从整体上把握疾病的本质,而应该努力寻找不同机制之间是否有着内在的联系,综合探讨运动神经元病的发病机制,从而为临床治疗上提供可靠的依据。 OBJECTIVE: To sum up and analyse the relevant hypothesises of motor neuron diseases (MND), and synthetically discuss the mechanisms of MND to offer the reliable basis for clinical therapy. DATA SOURCES: An online search of PUBMED database from January 1994 to December 2001 was conducted to consult the articles about MND with the key word "neurodegenerative diseases" in English. STUDY SELECTION: All the materials were firstly checked, the selecting standard: about the mechanism of MND; the eliminating standard: the reduplicative research. DATA EXTRACTION: There were 43 suitable articles in accordance with the above standards, and 24 of them were excluded due to the reduplicative contents. Only 19 articles were included. DATA SYNTHESIS: The main relevant hypothesises of the mechanism of MND are oxidative damage of free radicals, glutamate excitotoxic injury, inherited factor, immunity factor, lack of nerve nutrition factor and the injury of nerve fiber. Different hypothesises have their own evidence and research development. CONCLUSION: To research different hypothesises of the MND mechanism isolatedly have no good to master the nature of MND. We should make effort to find the inner connections of different mechanisms and synthetically discuss the mechanism of MND to offer the reliable basis for clinical therapy.
