改良眼袋整复术200例临床疗效分析

Clinical effect of modified baggy eyelid plasty: a report of 200 cases

目的探讨提高眼袋整复术的手术效果。方法对200例实施眼袋整复术者,术前采用“皮肤夹捏法”确定去皮量,术中按照预先设计先切除皮肤。局麻药中加大肾上腺素浓度达1∶14300,使手术野清晰。术中解剖出眶内侧脂肪球动脉,电离子去除眶脂体,避免眶部打结,使下睑平整,减少或避免球后血肿发生。术后即刻冷敷3小时,减轻术后水肿、淤斑。结果术后发生暂时性睑外翻1例,淤斑2例,无球后血肿发生,受术者均达到下睑平整,肿胀轻微,下睑松弛皮肤去除满意的效果。结论通过对眼袋整复术者围手术期关键步骤的改变,可明显降低眼袋整复术者并发症的发生率,提高眼袋整复术的疗效。

Objective To investigate clinical therapeutic effects after a series of improvements on baggy eyelid plasty. Methods Before operation the size of skin excision was defined and marked. The skin was excised according to the planned line. Adrenalin with the concentration of 1 ： 14300 was used during operation. The periorbital fat and bulbar artery were dissected. The local skin was given cold compress for 3 hours instantly after operation for alleviating local dropsy and ecchymosis. Results Among the 200 cases, 1 case had temporary ectropion and 2 cases had ecchymosis. Retrobulbar hematoma was not found in any case, and the results were satisfactory. Conclusion A series of improvements on operative techniques can enhance the clinical therapeutic effects of baggy eyelid plasty, and reduce side effects.
