脑动静脉畸形脑血流SPECT显像及其临床相关因素分析 被引量:1

摘要 为探讨脑动静脉畸形(AVMs)血液动力学变化及其与临床的关系,将健侧、病灶侧相应部位脑组织放射性计数值之差值除以该区域象素值(即象素总数),作为脑 AVMs 脑缺血指数,并与病人年龄、性别、临床症状和病变大小进行比较。结果:①脑 AVMs 并发癫痫与脑血流量减少有明显关系,而并发脑出血的脑 AVMs 脑缺血较轻;②脑血流 SPECT 显像评价脑 AVMs 产生的缺血优于病变形态学变化评价脑血流量改变;③脑 AVMs 产生脑缺血的程度与临床症状有关,与病人年龄、性别以及畸形血管大小无明显关系。故脑血流量 SPECT 显像对评价脑 AVMs 的血流量变化有重要意义。 PURPOSE To evaluate the relationship between clinical related events and rCBF SPECT imaging in patients with arteriovenous malformations (AVMs).METHODS The radioactive counting difference between normal and lesion site was divided by regional pixal considered as ischemic index (Ⅱ).Ⅱ was measured in 20 AVM cases and compared with patients' age,sex, neurological history and the size of lesions.RESULTS The degree of rCBF reduction correlated with clinical neurological manifesta- tion and showed no significant relationship with the age,sex and size of malformed vessels.Ⅱ in patients with seizures was higher than that in patients with hemorrhage.CONCLUSIONS The rCBF SPECT imaging may be useful for evaluation of the hemody- namics in AVMs.
