模拟战伤应激性脑缺血大鼠尼莫地平、音乐协同高压氧治疗观察

摘要 目的通过观察尼莫地平、音乐协同高压氧(HBO)综合治疗电击、创伤产生的应激性缺血性脑血管病,进一步探讨HBO、尼莫地平、音乐在缺血性脑血管病(ICVD)治疗中的作用。方法用电击、创伤模拟战时伤亡刺激大鼠视、听觉,产生应激,再结扎双侧颈总动脉(A),阻断血流20min后恢复血流灌注,给予HBO+音乐(应激1组)、HBO+尼莫地平+音乐(应激2组)和HBO(应激3组)治疗,检测血小板-CD62p(platelet-selection-CD62p,CD62p),红细胞内钙(intraerythrocytic-calcium,IECa2+),红细胞变形指数(dyformity-index,DI),皮质醇(cortisol,Cor)。断头取脑作形态学观察,并与对照组对比。结果应激1、2、3组大鼠CD62p、IECa2+、Cor含量明显高于对照组(P<0.05),以应激3组更显著(P<0.01),应激1组次之。DI值应激1、3组明显低于对照组和应激2组(P<0.05),以应激3组更显著(P<0.01)。脑组织形态学应激1、3组神经细胞数量减少,残存的细胞大部分肿胀,固缩、变性并崩解。应激2组仅见少数散在神经细胞肿胀、固缩、变性。结论尼莫地平、音乐配合HBO治疗有益于应激性ICVD的康复。 Objective To observe the effects of comprehensive treatment of hyperbaric oxygen(HBO), nlmodipine and music in the ischemic cerebrovascular disease(ICVD) which caused by electroshock and casualty in the war. Methods Using the electroshock and casualty simulated in the war to stimulate sight and hearing of the rats, causing hyperirritability. Then bilateral common carotid arteries of the rats were ligated and the blood flow was occluded. After 20 mimutes, the ligated arteries were loosen and the flow reperfused. At the same time, HBO + music( group 1 of hyperirritability), HBO + nimodipine + music( group 2 of hyperirritability) and HBO(the group 3) were given respectively. Then platelet-selection-CD62P(CD62p), intraerythrocytic-calciurn ( IECa^2+), dyformity-lndex(DI) and cortisol(Cor) were examined. At last, the heads of the rats were cut off and the brain tissues were taken out for morphology observation and compare them with control group. Resuits Compared with the control group, the levels of CD62 p, IECa^2+ and Cor of group 1, 2 and 3 of the hyperirritability were markedly higher( P〈0.05), especially group 3(P〈0.01). Among the three groups, group 3 was obviously higher than other two groups. DI of the group 1 and group 3 was lower than the control group and group 2(P 〈0.05), especially group 3(P 〈0.01). Morphological changes in brain tissues: the number of neuron cells decreased obviously in group 1 and group 3. Morever, most of the residual cells became swelling, pyknotic, degenerative or decompositive. These morphological changes were few in group 2. Conclusion HBO combined with nimodipine and music had great restorative effect on hyperirritability ICVD.
