心衰合剂对心力衰竭患者血管紧张素Ⅱ、心钠素和N终端脑钠素前体的影响 被引量:15

摘要 目的观察心衰合剂对心力衰竭(heart failure,HF)患者血浆血管紧张素Ⅱ(angiotensinⅡ,AngⅡ)、心钠素(atrial natriuretic peptide,ANP)和N终端脑钠素前体(Nterminal pro-BNP,NT-proBNP)的影响。方法59例HF患者(NYHAⅡ~Ⅳ级)随机分为两组,在卡托普利治疗的同时治疗组加用心衰合剂,对照组加用五苓散,疗程均为2周。观察心衰合剂对患者血浆AngⅡ、ANP和Nt-proBNP的影响。结果HF患者的血浆AngⅡ、ANP和Nt-proBNP随HF严重程度而呈递增趋势(P<0.01);治疗后两组血浆AngⅡ、ANP和Nt-proBNP较治疗前均显著下降(P<0.01),两组间比较差异无显著性(P>0.05)。结论心衰合剂对HF时神经内分泌过度激活有一定的干预作用。 Objective To observe the effects of Xinshuai Mistura (XM) on plasma angiotensin Ⅱ (Ang Ⅱ ), atrial natriuretic peptide (ANP) and N terminal probrain natriuretic peptide (Nt-proBNP) in patients with heart failure (HF). Methods Fifty-nine patients with HF (NYHA class Ⅱ-Ⅳ) were randomly divided into two groups, the treated group treated with XM and the control group with Wuling Pulvis (WP), both on the base of captopril treatment. The treatment course was two weeks, Plasma levels of Ang Ⅱ , ANP and Nt- proBNP were observed. Results The plasma levels of Ang Ⅱ , ANP and Nt-proBNP showed an ascending tendency along with the increasing in severity of HF( P〈0.01 ), and significantly decreased after treatment in the treated group (P 〈0.01 ), but with no remarkable difference as compared with those in the control group (P 〉 0.05). Conclusion XM had definite therapeutic effects on excessive neuroendocrine activation in HF patients.
