创伤晚期颈髓囊变的MRI表现与神经功能关系

摘要 目的:评价创伤晚期颈髓囊变的大小与创伤后神经功能的关系。材料和方法:患者伤后距MRI检查时间为7个月~13年。回顾性分析27例创伤晚期颈髓的MRI及临床表现。根据美国脊柱损伤学会(American Spinal Injury As- sociation,ASIA)1992年修订的Frankel分级标准进行神经功能评分,并将患者分为完全损伤组((Frankel A组,19例)和不完全组(Frankel B-D组,8例)。结果:完全损伤组的囊变长度、横截面大于不完全损伤组,而体积、ASIA评分小于不完全损伤组(P<0.05)。不完全损伤组的囊变横截面与ASIA评分间呈负相关(r=-0.72)。结论:创伤晚期颈髓囊变多发生在C_5~C_6水平,不完全损伤组囊变横截面的大小与神经功能改变有关。 Purpose: To evaluate the relationship between the size of the cervical cystic degeneration and neurological function of chronic cervical cord injury. Materials and Methods: The MRI scans were performed from 7 months to 13 years after injury. The MRI and clinic features of 27 cases with chronic cervical cord injury were analyzed retrospectively. The neurological function was graded with ASLA score, and the patients were divided into complete injury group( Frankel A group,19 cases) and incomplete injury group( Frankel B-D group, 8 case). Results: There was significant difference of the cystic length, cross section, vdume and ASIA score between the complete injury group and incomplete injury group(P 〈0.05). The ASIA score was not correlated with the cystic length, cross section and cubage in the cervical cords between complete injury group and incomplete injury group, but the ASIA score was correlated (r = -0.72). Conclusion: A cystic degeneration of chronic cervical cord injury is often seen in C5 to C6. The relationship between the cross section of cystic degeneration and the neurological function is correlated in the incomplete group.
