自体脂肪颗粒腔隙内注射移植修复面部凹陷 被引量:19

Local augmentation by lipoinjection into undermined cavity beneath the depression in the face

摘要 目的:探讨一种腔隙式注射移植的新方法用于自体脂肪颗粒移植来矫治面部凹陷畸形。方法:通过受区注射麻醉液预先估计填充量,在受区皮下脂肪层用细金属线锯切割使之完全分离形成袋状腔隙,以注射器抽取腰、腹部或大腿内侧脂肪颗粒并经纯化后超量30%注射游离移植于受区。随访2年观察脂肪移植后凹陷畸形的充填效果。结果:本组17例31个部位。充填术后未发现有感染、坏死、液化等症状。凹陷部位畸形消失,外观丰满,局部无硬结,质感同周围组织,未触及移植组织的界限。结论:本方法解决了自体脂肪颗粒注射移植在以往临床应用时因采用隧道式注射而发生的“卡壳”现象,影响了移植脂肪细胞的存活率及易造成局部高低不平的情况。该术式组织损伤小、并发症少、疗效确切。为自体脂肪移植提供一种新的方法和思路。 Objective To evaluate the effect of correction of local depression in the face by injecting autologous fat granules into an undermined cavity. Methods The demanding volume of fat suspension was determined according with the volume of Lidocaine injected just filling up the depression; and the cavity was made with a tinsel wirelike scalpel. Autologous fat granules obtained by liposuction from waist, abdomen, or femoribus internus were thus injected into the cavity, over 30 percent of demanding volume of fat suspension. All cases were followed up at least two years to evaluate the effect of correction. Results 31 depressions in 17 cases were corrected with our technique. No serious complications such as infection, necrosis, or liquefaction occurred after therapy. All depressions disappeared and the appearance looked good. Conclusion Injecting autologous fat granules into an undermined cavity is a good choice of correctting local depression in the face. With this technique, It is possible to solve the problem such as pinhole jam-up, uneven fat distributing, low fat survival in nowadays widely used tunnel technique.
