利尿剂治疗对老年高血压糖、脂代谢及肾功能的影响

Effects of antihypertensive by diuretic on glucos,lipid metabolism and kideney function in elderly hypertensive patients

目的探讨老年高血压利尿剂降压治疗对糖、脂代谢及血尿酸肾功能的影响。方法对46例老年高血压患者,口服利尿剂前、后测定血糖,血脂,血尿素氮,血肌酐,血尿酸,计算肌酐清除率并进行比较。结果血糖略有上升(P<0.05);血尿酸明显增高(P<0.01);总胆固醇、低密度脂蛋白胆固醇、甘油三脂无变化(P>0.05);血尿素氮,血肌酐增高及肌酐清除率下降与利尿剂剂量有关(P<0.01)。结论长期应用利尿剂治疗老年高血压应使用小剂量,减少对血糖、血尿酸及肾功的影响。

Objective To investigate effects of antihypertensive treatment by diuretic on glucose, lipid metabolism and kideney function in elderly hypertensive patients. Methods 46 cases of patients with elderly hypertension were chosen, their blood glucose, lipidemia and uric acid, BUN, creatinine were measured both before and after diuretic antihypertensive treatment, the creatinine clearance rate was calculated, and these parameters were compared and analyzed. Results Increasing of BUN and creatinine and decreasing of creatinine clearance rate are correlated with the dose of diuretic（P〈0. 01） ; blood glucose rises lightly （P〈0.05）, uric acid rises obviously （P〈0.01） ; total cholestery, low density lipoprotein-cholesterol and triglyceride did not change. Conclusion It is easy for elderly hypertensive patients with antihypertensive treatment combining with low dose diuretic in long term to reduce effects on glucose, uric acid and kideney function.
