妊娠中晚期合并外科急腹症23例临床分析

Clinical analysis of surgical acute abdomen complicated during the last two trimesters of pregnancy

目的探讨妊娠中晚期合并外科急腹症的临床表现和治疗方法。方法回顾性分析妊娠中晚期合并外科急腹症23例。结果急性阑尾炎13例,急性胆囊炎7例,急性胰腺炎3例。手术治疗17例,流产2例,早产2例,胎儿死亡1例,母体死亡1例。结论对中晚期妊娠合并外科急腹症的正确诊断有赖于对其解剖、生理变化及临床特点有足够的认识,及时诊断和早期手术可提高疗效。

Objectives To investigate the clinical feature, diagnosis and treatment of surgical acute abdomen complicated during the last two trimesters of pregnancy. Methods The presentattion symptoms, diagnosis and treatment of 23 patients with surgical acute abdomen complicated during the last two trimesters were retrospectively analysed. Results The diagnosis was as follows： 13 acute appendicitis, 7 acute cholecystitis, 3 acute pancreatitis. Operations were performed in 17 patients, the other 6 patients received conservative therapy. Abortion occurred in 2 patients, premature delivery in 2 patients , fetal death in 1 patients and maternal death in 1 patients. Conehmlon Correct diagnosis of surgical acute abdomen complicated during the last two trimesters of pregnancy depended on enough knowledge of the anatomy, physiology and clinical feature. Early diagnosis and operation may improve the outcome.
