肺鳞癌和腺癌组织与血液中T淋巴细胞亚群的对比观察

Observation of T-lymphocyte Subsets in Tumor Tissue and Peripheral Blood of Patients with Squamous Cell Carcinoma of the Lung

采用免疫组化技术检测３４例肺鳞癌和腺癌组织与血液中Ｔ淋巴细胞亚群。结果显示：肺癌组织比肺良性疾病组织中ＣＤ４＋／ＣＤ８＋细胞比值降低（Ｐ＜０．０１），提示肺癌组织局部的免疫机能低下。肺癌组血液比对照组血液中ＣＤ４＋／ＣＤ８＋细胞比值降低（Ｐ＜０．０１），但术后比术前明显增加，说明此组肺癌患者全身的免疫机能低下，切除肿瘤后免疫机能明显恢复。肺癌淋巴结转移组、低分化肺癌组比对照组组织中ＣＤ３＋细胞减少，提示肺癌淋巴结转移组癌组织局部的免疫机能更低下，肺癌分化程度越低，组织局部的免疫机能越低下。

In order to investigate the local and general immune function of patients with squamous cell carcinoma or adenocarcinoma and its clinical significance, we observed T-Lym-phocyte Subsets (T-LS) in frozen tissue and in peripheral blood specimens from 34 cases of squamous carcinoma or adenocarcinama with immunoperoxide-stained method. Result: the ratio of CD/CD cells in cancer tissue decreased significantly than those of the non-cancerous tissue (P<0.01). Conclusion: the local immund function in cancer tissue was at the lower state. The ratio of CD/CD cells and the amount of CD cell in blood of lung cancer patient were lower than those of non-cancerous disease and health control group (P<0.01), and the ratio of CD/CD cells in blood of patients who had tumor resected was higher than those of patients with tumor remained (P<0.05). The result showed that the general immune function of lung cancer patient was at a lower state, returnedto normal after removal of the tumor. We also found that cancer with reginal lymphoode metastasis and poorly differentiated carcinoma had lesser amount of CD cells than that of the non-cancerous group, although the amount of CD cells in blood of two groups showed no marked difference. CD cells in varying degree of cell differentiation also decreased in amount. This demonstrates that the local immune function in cancer tissue of patients with lymph node metastasis decreases even more, and the lower the cell differentiation, the worse of local immune function.
