小儿椎管内肿瘤临床特点分析

Clinical Characteristics of Intraspinal Tumors in Children

目的分析小儿椎管内肿瘤的临床特点,总结其诊治经验。方法分析26例椎管内肿瘤患儿的临床资料。结果26例患儿均行手术治疗。恶性肿瘤占54%,以神经母细胞瘤多见;良性肿瘤占46%,以囊性肿瘤和畸胎瘤多见。肿瘤70%位于胸腰段,多位于硬膜外。肿瘤全切除率为50%。70%的患儿术后症状完全消失或明显改善。复发患儿再次手术可获得良好效果。结论小儿椎管内肿瘤临床特点与成人存在较大差异,早期诊断、早期手术以及术后综合治疗是改善预后的关键。

Objective To analyze the clinical characteristics of intraspinal tumors in children and to summarize the experience in diagnosis and treatment. Methods The clinical data of 26 children with intraspinal tumors were ret- rospectively analyzed. Results All the patients were performed surgery. Malignant tumors accounted for 54% in which neuroblastomas were common, while benign tumors constituted 46% in which cystic tumors and teratomas were frequent. 70% of the tumors located at thoracic and lumbar region and 54% were epidural. Total removal was achieved in 50% and symptoms disappeared or were significantly improved in 70% postoperatively. Satisfactory results could be got by second surgeries if tumors recurred. Conclusion There is a big difference between children and adults in clinical characteristics of intraspinal tumor. Early diagnosis, early treatment and postoperative combined therapy are key factors to improve prognosis.
