大肠癌组织Bcl-2、增殖细胞核抗原定量检测与临床病理及预后的关系

摘要 目的探讨增殖细胞核抗原(PCNA)与原癌基因bcl-2在结直肠癌中的表达变化以及其与临床病理及预后的关系。方法应用图像分析技术及免疫组化方法,分别定量检测48例大肠癌、32例大肠腺瘤、18例正常肠黏膜PCNA与Bcl-2。结果PCNA的表达在正常肠黏膜、大肠腺瘤、大肠癌呈递增趋势,随癌分化程度的降低、伴有淋巴结转移,PCNA明显增高(P<0.05,P<0.01)。Bcl-2在大肠腺瘤和大肠癌中分别为79.54%±11.41%和65.78%±12.95%,两者均明显高于正常肠黏膜(P<0.01),癌又高于腺瘤(P<0.01);Bcl-2在高、中、低分化癌组织中逐渐降低,但无明显差异(P>0.05)。Bcl-2与PCNA无相关关系。结论大肠癌PCNA表达与其分化程度、淋巴结转移密切相关。Bcl-2的表达与癌的分化程度、淋巴结转移无关。定量检测大肠肿瘤中PC-NA和Bcl-2对判断肿瘤的生物学行为、预测淋巴结转移有重要意义。 Objective To investigate the expressions of proliferating cell nuclear antigen (PCNA) and pinto- oncogene Bcl - 2 and their clinicopathological significances in colorectal carcinoma patients. Methods The expressions of PCNA and Bcl - 2 were studied in 18 cases of normal colon mucosa, 32 cases of colorectal adenoma and 48 cases of colorectal carcinoma by the method (S-P method) and quantitative computed processed imaging analysis. Results The positive rate of PCNA expression tended to increase in a sequence of normal mucosa - adenoma - carcinoma, and evidently increased along with the degrading of cancer tissue differentiation and development of lymphatic metastasis. The positive expression rate of Bcl - 2 in colorectal adenoma and carcinoma was 79.54% ± 11.41% and 65.78% ± 12.95% respectively, higher in carcinoma than in adenoma ( P 〈 0.01), but was not related to the degree of cancer differentiation ( P 〉 0.05), nor associated with the expression of PCNA. Conclusions The MOD (mean optical density) of PCNA is correlated with the degree of differentiation and lymph node metastasis. PCNA and Bcl - 2 expression quantitative analysis is of important value in predicting the biological behavior and metastasis of colorectal carcinoma.
