异丙酚配伍米索前列醇用于人工流产镇痛效果临床分析

Clinical Analysis of Propofol Combined with Misoprostol to Relieve Pain in Induced Abortion

目的:评价舌下含服米索前列醇用于异丙酚人工流产镇痛对宫颈的松弛效果。方法:将要求无痛人工流产的早孕健康妇女分为两组。A组术前30min舌下服含服米索前列醇0·2mg,然后在异丙酚静脉麻醉下行人工流产手术;B组直接行异丙酚静脉麻醉下人工流产手术,对两组进行多项指标对比分析。结果:两组手术麻醉效果无显著差异(P<0.05)。但A组宫颈松弛度明显好于B组,手术时间也较B组明显缩短,且手术易于操作,术中出血量、术后阴道流血时间均明显的少于或短于B组(P<0.01)。两组均无人工流产综合征发生,但A组和B组分别有2例和3例发生呼吸抑制。结论:异丙酚静脉麻醉下实施人工流产术前30min舌下含服米索前列醇0.2mg,有较好的宫颈松弛作用,可增强子宫收缩,手术易于操作,术中出血少,术后阴道流血时间短,值得临床应用。

Objective： To evaluate administering propotol to relieve pain in induced abortion and observe the effect of misoprostol on relaxing cervix. Methods： Early pregnant women requested relieving pain in induced abortion were divided into two groups at random. Group A ： Misoprestol was set at the back arched roof of the vagina at 30 minutes before operation with a dosage of 0.2mg, followed with induced abortion by anesthesia with intravenous injection prepofol. Group B ： Induced abortion was carried out by administering prepofol for anesthesia with no preoperative medication. The data of the two groups were compared and analyzed. Results： It showed no significant difference in the anaesthesia effect and beelding volume between two groups （P 〈 0.05） . But the tension degree of cervix in group A was less than that of in group B, and group A had a shorter duration of operation compared with group B （ P 〈 0.05 ） . There was no syndrome accompanied with two groups, but there were 2 cases of respiratory depression occurred in group A and 3 cases in group B. Conclusion. Propofol is effective to relieve pain in induced abortion. And yet , it seems to be alert to bring respiratory depression under control for safety. The study suggests that misoprestol used to relieve pain in artificial abortion may intensify uterine contraction and make operating easier, and help to decrease operation blood loss and shorten the postoperative bleeding time. It is worth applying it clinically.
