摘要
目的:明确视网膜母细胞瘤基因信号通路的调节因子与口咽鳞癌的临床进展和预后的关系。方法:对一个95例口咽鳞癌的组织微阵列分别进行了Rb信号通路的相关因子,包括pRb、p16、CyclinD1的免疫荧光组化研究和一种新型的蛋白自动定量分析(AQUA)评估。结果:pRb、p16、CyclinD1的蛋白水平与局部复发率、无瘤生存率和总体生存率密切相关。在多变量风险回归分析中,无论5年局部复发率、无瘤生存率,还是总体生存率,高细胞核表达的Rb基因蛋白和Cyclin D1均被证实是有预后价值的生物因子。结论:视网膜母细胞瘤基因通路在口咽癌的发展演变中起到了重要的作用。该传导通路的相关因子有可能成为治疗口咽癌的靶标记物。
Objective: To determine the association between the components of Rb pathway and clinical outcome and prognosis of oropharyngeal carcinoma. Methods: Fluorescent immunohistochemistry for pRb, p16 and cyclin D1 was carried out on a tissue microarray of 95 oropharyngeal cancer cases with long-term patient follow-up data. For evaluation of protein expression, an automated in-situ quantitative measurement of protein analysis (AQUA) was used. Results: Protein levels of pRb and cyclin D1 were significantly associated with local recurrence, disease-free survival and overall survival. In multivariate Cox regression, high nuclear cyclin D1 and high nuclear Rb expression were proved to be independent predictor for local recurrence, disease-free survival and overall survival at 5 years. Conclusion: Our results indicate that the Rb pathway plays a vitally important role for the clinical outcome in oropharyngeal squamous cell carcinoma. Thus, the components of Rb pathway may be potential targets for molecular intervention in patients with oropharyngeal squamous cell carcinoma.
