冠心病患者血清脂联素和颈动脉内膜中层厚度的关系

Relationship between Serum Level of Adiponectin and IMT of Carotid Artery in Patients with Coronary Heart Disease

目的:探讨冠心病患者颈动脉内膜厚度(intinma-mediathickness,IMT)和血清脂联素(adiponectin,APN)的相关性。方法:将68例经冠状动脉造影明确为冠心病的患者,按冠状动脉狭窄程度分为3组,选择20例非冠心病的志愿者为对照组。分别测定血清APN及颈动脉IMT。结果:冠心病各组APN水平低于对照组(P<0.01),冠心病3组间APN水平差异亦有统计学意义(P<0.01);冠心病冠脉狭窄单支≥70%以上两组IMT较对照组明显增厚(P<0.01),而至少有一单支狭窄在50%～69%组IMT和对照组差异无统计学意义(P>0.05)。以冠脉血管狭窄程度的等级为因变量,APN、颈动脉IMT、TC、年龄为自变量进行多元回归分析显示APN水平和冠脉血管狭窄程度的等级呈负相关,颈动脉IMT和冠脉血管狭窄程度的等级呈正相关;以颈动脉IMT值为因变量,APN、TC、年龄为自变量进行多元回归分析结果APN水平和颈动脉IMT呈负相关。结论:在冠心病患者中血清APN水平和颈动脉IMT有很好的负相关性,同时,将两项指标相互结合可以有效地判断冠脉狭窄程度。

Objective： To investigate the relationship between the serum level of adiponectin （APN） and the intinmamedia thickness （IMT） of carotid artery in patients with coronary heart disease （CHD）. Methods： Sixty-eight patients with CHD were divided into three groups according to the severity of coronary artery stenosis by coronary angiography. The levels of APN and IMT in patients with CHD were measured and compared with those in 20 healthy adult volunteers. Results： The levels of APN in patients with CHD were significantly lower than those in control group （P 〈 0.01）. The IMT of carotid artery in two groups （degree of coronary artery stenosis ≥70%） were significantly thicker than those in controls （P〈 0.01）. The serum APN concentration was negatively correlated with IMT of carotid artery and the severity of coronary artery stenosis. The IMT of carotid artery was positively correlated with the severity of coronary artery stenosis. Conclusion： The levels of APN were negative related to IMT of carotid artery. Measurement of APN levels and the IMT together could be a good index in evaluating the severity of coronary artery stenosis.
